LifeWise of Washington
Wise Essentials Rx
Great basic coverage. A good option when you’re looking for
a low rate and more with up front coverage for routine care
needs. This plan provides up front coverage for your
first six office visits and coverage for generic
prescriptions.
Wise Essentials Copay
The ease and predictability of a copay. This plan also
offers higher annual deductibles which lowers your monthly
rate. It covers your first three office visits with a $25
copay, preventive care and more.
Wise Savings
A Health Savings Account (HSA) plan. Quality healthcare
coverage and the opportunity to save money on a pre-tax
basis for future qualified medical expenses. Make your money
work for you with a WiseSavings HSA.
Wise Choices Prime
Offers the widest range of covered benefits. A benefit-rich
plan that includes coverage for pharmacy, vision care and
maternity. A nice option if you’re looking to cover
all your bases.

Evolve Core
A catastrophic health plan that includes your first four
office visits with a copay of only $35 each, not subject to
deductible or coinsurance. Preventive care with no
benefit limit. Back to basics means you don't have to
pay for what you don't need, like maternity, prescriptions
and vision.
Evolve Plus
Similar to Evolve Core above but with Expanded coverage
including $25 per office visit copays, upfront radiology and
laboratory services included. Preventive care and
vision. Coverage for prescriptions included.
Evolve HSA 50
Evolve HSA 80
Evolve HSA 100
Simple plan design may be used with an HSA to help you
save even more. Choice of coinsurance limits from 50%,
80% and 100% offer lower rates with slightly more risk
sharing. Prescription coverage offered subject to
deductible and coinsurance.

KPS Health Plans
Sound Harbor Elite
Comprehensive PPO coverage with a $1,000
deductible per person. Coverage includes maternity, prescription drugs,
acupuncture, spinal and extremity manipulation, mental health, and massage
therapy. The deductible is waived for preventive care.
Essential Plus
A catastrophic PPO Plan that provides coverage for major medical events as
well as 100% coverage for the first three office visits after a $30 per visit
copay within the calendar year. In addition, the deductible is waived for
preventive care. This plan also includes generic prescription drug coverage.
Sound Harbor Essential
Catastrophic PPO Plans with a
choice of deductible and coverage for major medical events. The deductible is
waived for routine physical exams, mammography and prostate cancer screening.
Included at no cost to you is our Pharmacy Discount Program that allows you to
receive a discount at the time you purchase your prescription drugs. Worried
about a high deductible? KPS will waive your deductible in the event of an
accidental injury.
The Healthy Investor™
Plans
Plans offer lower premiums and
a chance for you to take charge of your health care spending through wise use of
a Health Savings Account (HSA). With the Healthy Investor ™ you can choose
between a plan with prescription drug coverage or one with our Pharmacy Discount
Program. This program will allow you to receive a discount at the time you
purchase your prescription drugs; however, the overall savings may vary
depending on the drug.

Catastrophic Plans
Best Value. Offers the most number of
choices between deductibles and coinsurance from any
company. Office visit copays, maternity, and
Accident benefit coverage that pays the first $2000 per
person per accident, even if you
have not met your deductible yet. With a $3.0
million dollar lifetime benefit that may be increased to
$6.0 million for around $3 per month, and up to a
$500,000 transplant benefit, this plan offers the highest limits of of
coverage available. Optional Prescription coverage is also
available with this plan.
Comprehensive Plans
Offer first dollar benefits
for office visits, with a $1500 dollar deductible, a
$2.0 million dollar lifetime and also a $500,000
transplant benefit maximum. Maternity coverage is
included with this plan.
HSA Plans
Offers great tax savings with a first
dollar benefit for preventive care of up to $500 per
calendar year, per person.
