• WiseChoices plans offer you a no-deductible option along with two deductible options. You can choose between 20% and 30% coinsurance (with preferred providers), depending on the deductible option you select. You pay a $30 copay for office visits (including naturopathy) and a $25 copay for alternative care office visits, when you use preferred providers. You never pay a deductible for these services.

   

• WiseEssentials is our catastrophic (or value) coverage plan. It provides coverage for major medical events as well as six office visits with the deductible waived and no out-of-pocket costs for preventive screenings. You can choose from three deductible options, and your coinsurance amount is always 25% (with preferred providers).

   

• WiseSavings is a high-deductible health plan qualified to work with a Health Savings Account (HSA). You open, contribute to and own the Health Savings Account so you can save money to pay for future qualified medical expenses. WiseSavings offers you two deductible options and 20% coinsurance (with preferred providers).

   

  Family Maximum is 3x Deductible $2.0 Million Policy coverage maximum.$250,000. Lifetime transplant benefit limit. Maternity coverage with Wise Choices Plans. Covered for preventive office visits and screenings. Over  16,000 healthcare providers $5000 Ambulance Limit.

• Breakthru Plans Coverage begins on day one. You can visit the doctor for a simple copay before using your deductible.  Preventive care for everyone is standard, not something you pay extra for. Vision care: you pay $30 for an annual exam and receive up to $200 for glasses and/or contacts.
• HSA Plans from Regence Own your health care dollars with a tax-advantaged account that covers medical expenses beyond your health plan. Or choose to save.
  More robust coverage than most HSAs, such as maternity and prescription coverage, plus a lower deductible.

Assurant Health Insurance

• Catastrophic Plans offers the most number of choices between deductibles and coinsurance from any company.  Office visit copays, maternity, and Accident benefit coverage that pays the first $2000 per person per accident, even if you have not met your deductible yet.  With a $3.0 million dollar lifetime benefit that may be increased to $6.0 million for around $3 per month, and up to a $500,000 transplant benefit, this plan offers the highest limits of of coverage available.  Optional Prescription coverage is also available with this plan. 
• Comprehensive Plans offer first dollar benefits for office visits, with a $1500 dollar deductible, a $2.0 million dollar lifetime and also a $500,000  transplant benefit maximum.  Maternity coverage is included with this plan.
• HSA Plans offers great tax savings with a first dollar benefit for preventive care of up to $500 per calendar year, per person.
